Pennsylvania law requires any student enrolling in a PA Public School, the parent must provide a copy of the child's Birth Certificate or another approved form of proof of age. A copy of the student's immunization records must also be supplied.
The parent will be asked to fill out a Registration Packet. All forms and records from the previous school must be received by JAHS before the student can start.
If possible, please bring copies of the student's records (transcripts, schedule, attendance, discipline) from the previous school with you at time of registration.
Be sure to make the school aware of any special needs/medical problems your child may have so that the school can provide proper adaptations for them.
If possible, please provide JAHS advanced notice of withdrawal.
Students should return all books, pay all monies owed, and clean out their locker(s) before withdrawing.
A parent is asked to come to the school to sign a withdrawal form and a release of records. If requested, the office can get a packet of student records ready for them to take to their new school to expedite their enrollment at their future district.
